1. View your Lab Requisition
Once your order is approved, from the 'Labs' tab in your portal, you will see an option to "Download" your lab requisition. A copy will also be emailed to you.
⚠️ Your lab requisition will be automatically issued for Quest Diagnostics. If there is no Quest location in your area, please contact our team, and we will provide a LabCorp requisition for your convenience.
If you are located in NY, NJ, RI, or HI, please note that we cannot automatically issue lab requisitions due to state regulations. If you reside in one of these states, please contact our team and we will work with you to find a suitable solution for labs in your area.
2. Schedule Your Appointment
Secure your testing appointment swiftly through Quest or Labcorp booking system, accessing over 2000+ partner labs nationwide.

3. Preparation Guidelines
- Fasting: Do not eat or drink anything except water for 12 hours before your test.
- Exercise: Avoid any strenuous physical activities for 12 hours prior to the test.
- Hydration: Drink plenty of water to stay hydrated, but avoid other beverages.
Some individuals may feel light-headed or dizzy after a blood draw. It's advisable to bring a small snack or drink to consume afterwards to help you feel more comfortable and stable.
If you are undergoing the Longevity Pro test, keep in mind that while the blood volume drawn is slightly more than usual, it is still a safe and routine amount.
4. Visit Your Lab Location
After scheduling your lab visit, be sure to bring your lab requisition form with you. You can simply show it on your phone, so there’s no need to print it out.
When you check in, let the staff know your visit has already been prepaid and that no payment is required.
5. Lab Results Delivered To Your Patient Portal
After your sample has been collected at the lab, please stay tuned for updates on your results through your patient portal. You will also receive text notifications to ensure you never miss an update. Depending on the lab, this can take up to two weeks.
